Title: newspaper combine--two entrances or one?

I did a search and did not see an answer to this.  If I place one deep on a deep and a half hive with newspaper in between do I make sure they have a top entrance or simply the bottom entrance?

Also, can I, later, put the medium on top of the two deeps once combined or should I do this at the time of the combine making it go: deep--paper--deep--paper--medium?

Give the new box of bees no entrance, but cut some slits in the paper.

What I do depends on the weather.  It's hot here today.  I'd give them both entrances.  If it was cool I wouldn't worry about it.
