How to combine hives?

I'm opting for the newspaper method. From what I've gleaned from all the information floating around. The queenless hive goes on top and two or three sheets of newspaper are placed between the two hives.
My questions are:
Do I maintain the entrance for the top hive? If so how?
If not, it seems that this may cause a ventilation problem??
Do I put the top feeder on? (The queenright hive is no longer using a feeder)
I've heard a lot of success stories but not a lot of details.

I use one sheet of paper with a couple of cuts from the hive tool.  As for the one on top I put the one that is being moved.  for top enterence if the hive body doesn't have a hole drilled i add a 3/4" shim with a hole or slot

Curious, if I caught a small swarm and I know I didn't get the queen, I add them above a hive to combine, can I trap them in their super without an entrance? So the only way out would be to eat through the newspaper and out the bottom entrance.
