Forms for protection on cutouts?

Hello folks, I was wondering if anyone knows where I can find a form that I can use when I do cutouts to help protect me. The reason I'm asking is I have a cutout coming up but would like to have something signed by the home owner so they know when doing a cutout you don't know to expect. Any info would be greatly appreciated.

Bigfoot do a search for waiver.  I had asked this same question not long ago and recieved a great response.

So where does one find a Hold Harmless Indemnification Form?  Do you have to go to a lawyer?
